The Colorado Opioid Synergy for Larimer and Weld counties is a network of MAT clinics that offer medication assisted treatment and counseling services. The program offers access to Naloxone to anyone who needs it.

Health District of Northern Larimer County
Offers 4 different programs geared towards mental health. Connections provides support for individuals and families looking for help with mental health or substance use concerns; CAYAC offers help to young people (up to age 24) and their families with identifying and treating mental health and substance use concerns; classes, trainings, and talks for the public on topics surrounding mental health; and Integrated Primary Care and Health Program that integrates mental and physical health at UCHealth and Salud Family.
SummitStone Health
SummitStone provides more than 50 services dedicated to treating mental health and addiction disorders for people of all ages. Fort Collins and Loveland locations.

Innovage offers a Program of All-inclusive Care for the Elderly (PACE). Each participant works with a team of medical experts to develop a care plan that is personalized and regularly updated to meet their evolving medical and social needs.

Salud Family Health Centers
Salud Family Health Centers has 13 clinic locations and has an enrollement specialist to help with reducing costs and enrolling in insurance at each location. They focus on serving low-income, medically underserved populations as well as the migrant and seasonal farmworker population. Salud accepts Medicaid, Medicare, CHP+, and most private insurance plans. They also offer a sliding fee scale based on family size and income.
